As a courtesy we will bill your primary insurance company for the services we provide.

Please bring your insurance card to each appointment so we may keep your record current.

Please notify the business office immediately if your insurance company sends you a new card or if you change insurance companies.

If you have not met your yearly deductible, you will be responsible for payment at the time of service.

If your insurance requires you to pay a co-payment or co-percentage, the appropriate amount will be due at the time of service.

Since we are a specialty practice, please contact your insurance company to determine if a referral from your primary care physician is needed.

If your insurance company does require a referral, we must have the authorization in our office prior to the date of service.
